If you are an uninsured or self‑pay psychotherapy client of Reimagined Mind Counseling & Consultation, you have the right to receive a Good Faith Estimate of expected charges for non‑emergency services under the No Surprises Act.cms+2

• You can request a Good Faith Estimate before scheduling or at any time during your care.

• If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill.

This brief notice does not replace the full Good Faith Estimate that will be provided to you upon request or at the time of scheduling.
